Describe Project/Event:

Maisha International Standards
1. All project and event fundraisers must be submitted to and approved by Maisha International.
2. Any Maisha International name or logo usage must be preapproved by Maisha International.
3. Drafts of fliers, invitations, banners, T-shirts, etc. with M.I.O name or logo must be preapproved.
4. All projects must state Maisha International Orphanage is the “beneficiary”, not the “sponsor”.
5. Maisha adheres to a high moral conduct and requests all fundraising coordinators and teams to exhibit the same high morals.
6. All projects must meet all ordinances and laws set forth by the city, state and federal government.
7. Maisha International Orphanage does not assume responsibility or liability for accidents or losses during the event or project.

I understand by submitting this application I am not guaranteed approval. My fundraising project will be reviewed and I will be notified within one week of my approval/decline. I have read and will comply with Maisha International Orphanage standards as stated above. I release and hold harmless Maisha International Orphanage from any and all responsibility and liability surrounding this project/event.
